Kindergarten requirements

  1. Birth Certificate- Proof of age – Upon enrollment for the first time in any elementary or secondary school, a student or student’s parent shall provide a certified copy of the student’s birth certificate.

  1. Immunization Certificate-A current Kentucky immunization certification form required by 704 KAR 4:020 shall be on file within (2) two weeks of a student’s enrollment in school. Immunization Certificate

  3. Social Security card-Copy of the student’s Social Security card.  

To obtain a Social Security Card application, go to www.socialsecurtiy.gov or call 1-800-772-1213 for assistance.

  1. Eye exam-requires proof of a vision examination by an optometrist or ophthalmologist.  This evidence shall be submitted to the school no later than January 1 of the first year that a three (3), four (4), five (5) or six (6) year old is enrolled in public school, public preschool, or Head Start program. 
  2. Dental exam-requires proof of a dental screening or examination by a dentist, dental hygienist, physician, registered nurse, nurse practitioner, or physician assistant to be presented to the school no later than January 1 of the first year that a five (5) or six (6) year old child is enrolled in public school. http://www.monroe.kyschools.us/docs/DentalScreeningFormasapprovedbyBoardofEducation.doc.pdf

6th grade

9th grade

  2. Up to date immunization certificate-

New student to the district other than above

  3. Physical -The preventive health care exam (physical) dated within I year prior to initial admission to school and must be on the Preventive Health Care Form.
  4. Social security card-Social Security Number due upon enrollment.
  5. Eye exam-All children entering at 3, 4, 5, or 6 years of age public preschool and Head Start or public school for the first time to have an eye examination by an optometrist or ophthalmologist no later than January 1 of the school year.
  6. Dental screening- Effective with the 2010-2011 school year, a dental screening or examination shall be required for five (5) and six (6) year olds enrolling in the school district.
